Medication Administration by school employee or by student requires a completed medication permission form to be on file for each school year.
All medication will be destroyed once expired or after the end of the school year if not picked up to ensure the safety of all students.
Students may not carry medication on their person without proper documentation.
Management of asthma by either the student or school employee requires a completed asthma packet each year.
Seizures, Diabetes, Life-Threatening Allergies: (food, insects, Latex, other)
If your child has one of the above medical conditions or any condition in which the school staff may need to provide emergency response or support, please contact the district health coordinator at 869-8090 and complete a medical packet.
