Health Forms

Health Services Last Updated: 7/29/2020 3:19 PM

Medication Administration by school employee or by student requires a completed medication permission form to be on file for each school year.

Policy Overview:

  • All medication MUST be brought to office by the parent to be signed in.

  • All medication will be destroyed once expired or after the end of the school year if not picked up to ensure the safety of all students.
  • Students may not carry medication on their person without proper documentation.

  • All medication must be brought to school in the original container.  Labels that have been altered or received in alternate container will not be accepted.

  • Medications can not be repackaged for field trips, so a new bottle with an adequate amount of medication for the trip will need to be brought to the school with a current pharmacy label.

Management of asthma by either the student or school employee requires a completed asthma packet each year.
